Zunehmende Luftverschmutzung infolge fortschreitender Industrialisierung, ansteigen der Tabakkonsum und Erhöhung des durchschnittlichen Lebensalters haben die Erkrankungen der Atmungsorgane erheblich vermehrt. Im klinischen Gesamtkrankengut ist heute mit 6-8% solcher Krankheitsfälle zu rechnen. Dabei ist vom 50. Lebensjahr an ein steiler Anstieg festzustellen. Bei Patienten über 50 Jahre findet man in 33%, bei Patienten über 60 Jahre in 53% manifeste Erkrankungen des respiratorischen Systems. Im präoperativen Status sowie intraoperativ nehmen respiratorische Störungen in der Skala der begleitenden Morbidität den zweiten Platz ein, postoperativ ftihren sie mit weitem Abstand die Liste der Komplikationen und letalen Verläufe an. Insgesamt stel len die Beeinträchtigungen der Lungenfunktion den relevantesten und bedrohlichsten pathophysiologischen Einzelstörfaktor ftir den operativen Patienten dar. Diese Tatbestände ließen die Veranstaltung eines Workshop begründet erscheinen. Einschlägig erfahrene Wissenschaftler und Kliniker sollten aus verschiedener Sicht ihr Spezialwissen einbringen; die Synopsis ihrer Erkenntnisse und Erfahrungen sollte eine möglichst umfassende Abklärung des Risikos respiratorischer Störungen ftir den Patienten in der Anästhesie ergeben. Zunächst war die Frage zu klären, wie das heute verfügbare breitgefacherte Unter suchungsprogramm der Lungenfunktion einzusetzen ist und welche Aussagekraft die Einzelkomponenten besitzen. Die synoptische Analyse sollte Aufschluß geben über den Stellenwert folgender Untersuchungsmethoden: Gezielte Anamnese und klinische Befunderhebung, die Untersuchung der "kleinen Lungenfunktion" mit ihren Meß ergebnissen der Vitalkapazität, des Sekundenvolumens, des Atemgrenzwertes und diverser Flußraten in verschiedenen Exspirationsphasen, die weitergehende Abklärung der statischen und dynamischen Lungenvolumina wie der Strömungswiderstände in den Atemwegen mittels des Ganzkörperplethysmographen, die Ergänzung durch die Messung der Blutgase an verschiedenen Orten des großen und kleinen Kreislaufes.
