Alla ballerina professionista Nina Sayers, dopo anni di sacrifici e costante applicazione, si presenta l'occasione della vita quando il direttore artistico Thomas Leroy le assegna il ruolo della regina de "Il Lago dei Cigni" di Cajkovskij. Lei è perfetta come angelico cigno bianco, ma non sufficientemente perversa come cigno nero, ruolo più adatto alla sensuale e spregiudicata Lily, con la quale si instaura una psicologica rivalità che porta Nina ad esplorare i propri sentimenti più profondi e oscuri. Mentre Nina fa di tutto per raggiungere ossessivamente quella perfezione da sempre inseguita, gli incubi, le gelosie e le fantasie inconsce iniziano a farsi strada in maniera profonda e ad insinuare in lei un irrisoluto dubbio: quello con Lily è un conflitto reale o solo frutto della propria mente? I conflitti interiori e gli impulsi oscuri prenderanno lentamente il sopravvento sulla psiche di Nina, trasformandola da una giovane ingenua in una creatura instabile, che in breve tempo riuscirà a calarsi fin troppo bene nel ruolo del malvagio e autodistruttivo cigno nero.
